TAILS IN THE CITY requires all clients to complete a Veterinary Release Form.

In the event of an emergency. TAILS IN THE CITY will make every attempt to contact the owner(s) and/or emergency contact. In the event that no contact can be reached, TAILS IN THE CITY will seek appropriate medical care for your pet(s).

TAILS IN THE CITY will make every effort to take your pet(s) to the Veterinarian listed below; however, if your Veterinarian is not available, TAILS IN THE CITY will bring your pet(s) to the nearest emergency clinic.

At Tails In The City, the safety and well-being of all pets in our care is our top priority. By using our services, you confirm that your dog/pet is in good health and has not exhibited aggressive or threatening behaviour toward people or other dogs. This ensures a safe and enjoyable experience for every pet in our care.

In the case of an emergency, I understand that TAILS IN THE CITY will make every attempt to contact the owner(s) and/or emergency contact listed in this form. If those listed in this form cannot be reached, I authorize TAILS IN THE CITY to seek appropriate medical treatment for my pet(s).

I understand that every effort will be made to take my pet(s) to the above Veterinarian; however, I authorize TAILS IN THE CITY to seek treatment for my pet(s) at any appropriate veterinary clinic. If necessary, I give permission to TAILS IN THE CITY to approve treatment up to the amount indicated above.
